NIFS Membership Cancellation Request Form
 
  • A cancellation is not effective until a signed Cancellation Request Form has been received and processed. Your cancellation date is based on the date your cancellation request is submitted and processed, not the date of your last visit to the Fitness Center. Please read our cancellation policy below and then complete the form on the right.
  • Payments already processed are non-refundable.
  • If your account is not in good standing, any balance will be due at the time of cancellation (including any unpaid join fee installments and club account charges) or your AutoPay will remain active until a zero balance has been reached.
  • Annual dues payers may be issued a refund for unused future months less a $20 processing fee.
  • Refunds will not be issued due to non-usage of the facility.
  • To rejoin, former members pay only $50 as long as a prior join fee has been paid in full.
  • You will receive a personal confirmation within 2 business days after submitting your online Cancellation Request Form. Save your confirmation email as a receipt in case it is needed to verify your cancellation.
